Kiwanis Club of Lake Tahoe Youth Organizations Financial Request Form for donations to the South Shore Lake Tahoe Community. 
 
There was one report that an uploading problem occurred using the Safari browser. If that situation happens to you, try switching browsers. It worked for the person who reported the problem. :)
 
1. Fill out the Request for Funds/Support Form click on the - Kiwanis Club of Lake Tahoe Youth Organizations Financial Request Form
2. Save a W-9 Non Profit Taxpayer Identification Number (TIN) on a computer so you can upload/attach to the Request for Funds/Support online application (the very last item on the application). 
3. Review: You will be submitting both the Request for Funds/Support Form and W-9 at the same time/same online form.
